What should I do to prepare for my first visit?
You should complete the quick digital intake form that will be sent via email prior to your first appointment. (Sometimes the digital intake form goes to SPAM or Junk mail, be sure to check.)
Please bring any relevant medical info including doctor prescriptions, Imaging reports or surgical reports you might have.
Do you accept my insurance?
Integrated PT² is a out-of-network practice with all insurances.
To check if we work with your insurance please contact us by calling (917) 520 – 1708 or by filling out the insurance verification form to the right and we will return your request within 24-48 hours. There are no hidden fees and you will know what your responsibility is prior to your first appointment.
How long should I allow for my appointment?
At Integrated PT² appointments are about 1 hour and all appointments are one-on-one to give you full attention.
What should I wear to my first appointment?
You should wear clothing that would allow access to the area of concern. Generally, loose fitting or athletic wear is worn for comfort and function.
Are there showers and changing rooms?
Yes, we have a few showers, towels and basic amenities that are available at Integrated PT².
